How do I swtich my business from a proprietorship to an LLC?

I have a small business that has been open since August of last year. We are now ready to transition from the proprietorship to an LLC. I was wondering what i'll need to in order to make this happen. The business is in Oregon. I'd like to know any advantages of doing this or any other information would be appreciated.
